If you clicked a billing link from our team, here's how to reach the Billing Requests form:
1. Click “Client Login” above and sign in with your portal credentials.
2. Once inside, select “Billing Requests” from the left-hand menu.
3. Fill in the short form.
4. Click “Submit request” and our billing team will follow up directly.
